PHOTO / VIDEO RELEASE

I hereby grant permission to akorn environmental consulting, LLC to reproduce any portion of the photo images and video files submitted through this form for purposes which can include but are not limited to: books, cards, calendars, invitations, white papers, reports, peer-reviewed publications, maps, newsletters, and websites without any more compensation or recognition given to me. Furthermore, I grant creative permission to alter the photograph(s) and/or video(s).

I do not grant permission to resale or use the photographs or videos in a manner that would exploit or cause malicious representation toward me or my company and associates or organizations whom I deem to be partners.

Additional Terms:

• I understand and agree that all photos and videos will become the property of akorn environmental consulting, LLC and will not be returned.
• I hereby irrevocably authorize akorn environmental consulting, LLC to edit, alter, copy, exhibit, publish, or distribute these photos and videos for any lawful purpose, including for commercial purposes.
• I waive any right to inspect or approve the finished product wherein my likeness or the photographs/videos appear.
• I waive any right to royalties or other compensation arising or related to the use of the photo or video.

Release of Liability: I hereby hold harmless, release, and forever discharge akorn environmental consulting, LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Warranties: I warrant that:

• I am the copyright owner or have permission to grant this license
• The photos and videos do not infringe on any third-party rights
• Any people identifiable in the photos or videos have consented to their use
• I am at least 18 years of age

Any infringements of this policy may violate federal law.
